Biometric screening results provide powerful insights into your health risks that you may not currently recognize. Complete your biometric screening to know your numbers and connect to quality care to help manage health risks and prevent chronic disease.
Step by step: How to schedule a screening
If you have not registered for the BeWell SHBP® program, register here.
If you have already registered for the BeWell SHBP® program, you can log in here.
Once logged into your Sharecare account, click on the “Achieve” link on the left- hand side of the page.
Click on the “Programs” tile.
Click on the “Health Screening” tile. This will redirect you to the Quest scheduler.
Read the Terms and Conditions and click “Accept & Continue”.
Read the consent language, click on “I accept” then click the “Continue” box.
Confirm your personal information and mailing address is correct and click “Save”.
To schedule a screening at a Patient Service Center (PSC) click on “Schedule a Screening” or to schedule a screening at an onsite event click on “Make an Appointment”.
Search for and select a desired location.
Choose desired date and time.
Review and confirm all information.
Who can complete a biometric screening?
Members and their covered spouses enrolled in Anthem Blue Cross Blue Shield, and UnitedHealthcare.
Potential earnings: $960 ($480/each)
What happens during an SHBP sponsored onsite screening event?
A health professional will collect measurements, including height, weight, waist circumference, and blood pressure.
Only the following is tested through a fingerstick: cholesterol, glucose, hemoglobin A1c, and triglyceride levels.
You will get a personalized report highlighting your screening results
Hemoglobin A1c results will be loaded into the Sharecare portal within 10 business days.
You get a detailed explanation of what your numbers mean and the healthy target ranges.
You will learn about areas for improvement and recommendations, if needed.
Is my privacy protected?
Yes. Your personal health information is held in strict confidence
and will not be shared with your employer.
What rewards can I earn for participating in the BeWell SHBP Well-being Program?
Eligible members and covered spouses can earn up to 480 points and choose to redeem them for either:
A $150 Sharecare Rewards Visa® Prepaid Card (to use anywhere Visa is accepted; expires within 12 months from when it was issued) OR
$480 well-being incentive credits (translates to dollars to apply toward eligible medical or pharmacy expenses)
Screenings are confidential, voluntary, and offered at no
additional cost to SHBP members or covered spouses.
